I understand by nominating this individual to the SCPS Fellows Program that he/she is not automatically enrolled into Loyola University Chicago. A Loyola admission representative will contact the student upon submission of this form and he/she must complete an application and interview process. You will be notified as to the status of this students acceptance.

I understand that employers are required to inform Loyola if there has been a change in employee status for an enrolled SCPS Fellow (such as termination of employment).
