This form is intended for prospective, domestic, adult learners seeking an online undergraduate bachelor's degree program and have not applied for admission to the School of Continuing and Professional Studies (SCPS).

Note: Prospective adult learners with international higher education credits must have their official transcripts evaluated by a NACES member organization. The evaluation needs to be a Catalog Match Evaluation for admission.

Please allow up to 5 business days for processing. The transcript evaluation is an unofficial document, and an error will not exempt you from a requirement.
