GPEM Admission Change Request Form

This form is intended for schools/programs to notify GPEM of changes to programs impacting the admission process. This is not for new programs/changes to existing programs that require Provost approval.

You will be notified of estimated completion date based upon current existing requests and also when the changes are in effect.  Please direct any questions to Maria Villanueva, mvilla2@luc.edu.
